Medication Pathfinder
A Resource for Documentation Referral and Teaching

Medication Pathfinder is a web-based Medical Management application, which allows pharmacists to produce medication therapy management (MTM) recommendations. Duquesne University Mylan School of Pharmacy adopted this program in 2008. Since then, the School of Pharmacy has expanded its use to fourteen parent accounts and now uses this program as a primary source of universal record keeping for patient interventions and documentation to provide a continuum of care throughout its network of clinical practice sites.

This unique program allows pharmacists to not only input patient information and update lab values with every visit, but it also customizes risk factor assessments to meet the needs of the facility. At Duquesne, the faculty, residents, and fellows, along with the staff at Medication Pathfinder have created customized assessment tools to evaluate patients more efficiently.

These tools not only provide efficiency at the patient level, but they are used as a teaching aid as well. Students in the PY4 experiential rotations are trained on Medication Pathfinder in several ambulatory care rotations with Duquesne faculty and utilize these programs to master various components of patient care. The students leave the rotation with a comprehensive understanding of documentation, including SOAP assessments, medication action plans, and drug regimen reviews. Medication Pathfinder also allows the student the opportunity to bill for patient visits using MTM codes.

Medication Pathfinder is a necessary entity when utilizing clinical pharmacy services. It allows a comprehensive medical assessment of a patient with thorough record keeping and accessibility for documentation of billing. It also provides a link in communication among physicians, pharmacists, and other healthcare providers. This program serves as a teaching model for experiential students at Duquesne University and faculty, residents, and fellows will continue to find an unlimited number of uses for this exceptional program.
